  • During the cardiac cycle (action potential), from beat to beat, the changes
  • There are 3 main causes of electrical alternans

  • Repolarization alternans
  • Conduction Alternans
  • Motion Alternans
    • Is the most common cause of electrical alternans
    • Occurs in pericardial effusion
    • The heart "swims" in the effusion
    • On the ECG, the amplitude of QRS complexes changes

Electrical Alternans

  • Electrical alternans is a general term
    • For changing (from beat to beat) amplitude of the ECG waveform
  • Amplitude of any part of the ECG waveform may vary
  • The most common cause is pericardial effusion
    • Presence of QRS alternans (variation in QRS complex amplitude)
