Interatrial Block

Interatrial conduction delay (IACD), Interatrial block

Bachmann's Bundle

Heart bachmann bundle (interatrial bundle)
  • It is the interatrial part of the cardiac conduction system
    • It runs from the SA node to the left atrium
      • Connects the right and left atrium
  • Conducts the impulse from the SA node to the left atrium
    • Ensures synchronous activation of the atria

ECG and Interatrial Block

ECG interatrial conduction delay (IACD), Bachmann block
  • Broad P wave > 0.12s
    • Has a notch in the middle (indicated)
  • PQ interval is not prolonged
  • For better visualization
  • On ECG, there is no exact boundary between
    • P mitrale and interatrial block
    • P wave in interatrial block has lower amplitude

  • There are 3 types of interatrial block

Types of Interatrial Block

Normal P Wave

ECG - Normal P wave and normal atrial activation

Partial Block

ECG - Partial interatrial (Bachmann) block, type 1

Complete Block

ECG - Advanced interatrial (Bachmann) block with left atrial retrograde activation, type 3

Interatrial Block



ECG with atrial premature complex and altering interatrial (Bachmann) block, type 2

Interatrial Block

  • Sinus Rhythm
  • Atrial Extrasystole with interatrial block
    • The impulse from the atrial extrasystole is partially blocked in Bachmann's bundle
      • Other P waves are normal
    • Low and broad P wave > 0.12s (with a notch) (red arrows)
    • This indicates a partial interatrial block (type 2)

Interatrial Block

  • Sinus Rhythm
  • Wide biphasic P wave > 0.12 (lead III)
  • This indicates an interatrial block (type 3)
